Other clinics visits policy
You have signed an enrollment form with your doctor which requires that you visit us for all of your health issues.
In case you visit another walk in clinic, this will lead to your doctor being penalized by the Ministry of Health.
Frequent visits to other clinics might be considered as a breach of the registration contract and can lead to removing your name from our practice.
Visits to hospitals, Emergency departments, Specialists and any visits outside Ottawa are not included in the above policy.
Scent Free Environment
In response to health concerns, Health Treasure Medical Center has developed a Scent-Free Policy. Scented products can trigger reactions such as respiratory distress and headaches. Patients are asked to not wear these products when visiting our office.
Some people may report sensitivities or allergies to various chemical-based or scented products. We ask for everyone’s cooperation in our efforts to accommodate their health concerns.
Unfortunately we are unable to accept any patient without a valid health card, please ensure that your health card is valid by checking the expiry date. To renew your health card please contact MOHLTC INFOline at 1-866-291-0609 or ServiceOntario Health Card Services-OHIP Office at 1-800-267-8097
We will contact you for a positive result and discuss your result over the phone, if you do not receive a call that means your doctor did not find any abnormality in your test and no need for you to come or to call.
We do not use emails for any kind of communication except for appointments.
We do not provide copy of test results, if you would like to get a copy for yourself you will be required to pay the fee.
Notarization and Guarantorship
We do not notarize documents or act as guarantors, please contact your lawyer for that purpose.
Inappropriate or violent behavior will not be tolerated under any circumstances. Doctors have the right to terminate patient physician relationship if patients act violently or inappropriately in the center.
